Agent Failure Mode

An agent failure mode is a specific, named way an agent can break and what follows: for example, hallucinating a field that does not exist, looping on a tool call, misreading an ambiguous CRM record, or sending outreach to the wrong segment.

Failure mapping catalogs these patterns with their consequences before deployment. It is distinct from reliability metrics, which quantify how often failures happen, and from recovery procedures, which describe fixes afterward.
