Form Intent Map

A Form Intent Map is a documented classification that assigns each form type to an intent tier—demo request, trial signup, content download, newsletter—and links each tier to its follow-up path: routing destination, response SLA, and nurture track.

It is the upstream decision that timing rules depend on.

A practical example

Example: the map places demo requests in tier 1 routed to sales with a one-hour SLA, trial signups in tier 2 routed to onboarding, and newsletter signups in tier 4 routed to a welcome nurture with no sales touch.

What to evaluate before investing

  • Can routing rules and SLAs be configured per form type rather than per form instance?
  • Does the platform let you document or label each form's assigned tier inside the tool itself?
  • When a new form is created, is assigning an intent tier a required step before it goes live?

Limitations and tradeoffs

Intent tiers can drift as offers change; a 'content download' that quietly becomes a sales-qualified demo request will keep tier-4 treatment until someone reclassifies the form and retests the routing.
