Form QA Gate

A Form QA Gate is an executable pre-launch checklist with named sign-off that every form must pass before going live.

Form Acceptance Criteria define whether a form's requirements are met; the gate is the enforcement step that catches broken field mappings, missing hidden fields, dead routing targets, absent spam protection, and tracking that never fires.

It exists because broken forms are usually discovered days later through lost leads, when the original launch context is gone and nobody owns the fix.

A practical example

Example: before a gated-report form launches, ops runs the gate: submits three test entries, confirms the hidden campaign field lands in the CRM, verifies the routing owner receives the notification, and signs off in the launch ticket.

What to evaluate before investing

  • Does the checklist include a live test submission traced end to end into the CRM with correct field values?
  • Is sign-off assigned to a named person recorded in the launch ticket, not a verbal approval?
  • Does the gate cover spam protection and tracking verification, or only fields and routing?

Limitations and tradeoffs

A gate slows launches by design; keep the checklist short and risk-based so teams do not bypass it under deadline pressure, which defeats its purpose.
